The Mobile Integrated Health Program integrates Community Paramedics, Outpatient High-Risk Case Managers, Medical Directors, and a dedicated vendor partner to extend care beyond hospital walls. Community Paramedics provide in-home clinical assessments, medication reconciliation, safety evaluations, and real-time escalation when needed. Outpatient High-Risk Case Managers ensure continuity of care, close gaps in follow-up, and address social determinants of health. Together, this collaborative team provides an added layer of post-discharge support that improves care transitions and patient outcomes.
